To submit your x-rays and receive a FREE X-Ray or MRI Evaluation, please call Laura Morelli at 1 (800) 881-8485 or email her at: fklaura@tampabay.rr.com.

If you do not have X-Rays you can request a prescription for an X-Ray be taken in your local area.

Patient Information Needed:

    * Patient history
    * Age, height, weight
    * Whether you have had injections?
    * Whether you are taking anti-inflammatory medication?
    * Whether you are diabetic?
    * Do you have any heart problems?
    * If you have had prior surgery?

X-Ray must be:

* Taken within the past 6 months.
* Weight Bearing Films

Mail films or CD to:

Once the films are received the doctor will review them and make recommendations.

Laura, will contact you with the doctors findings.
