• Please bring insurance card
• Please bring referral if needed. It is advised that you call your insurance company ahead of time to see if a referral is needed.
• Please bring all pertinent medical records

Payment policy

Health insurance is a contract between you and your insurance company. Copayments and deductibles are the portion of services that is paid out of pocket. All deductibles and copayments are due at the time of service. These payments are based on the patient’s insurance policy and cannot be waived.
